Your signature will be required on the placement agreement and the application so please download a copy of the placement agreement, and the application, sign it and place it in the mail along with a check for $10 payable to FLTAP.

This online application only serves as your intention to adopt and without a signed application and placement agreement, the adoption process may not continue. Once you submit this online application you'll be brought to another page where you can elect to pay your $10 non-refundable processing fee through Paypal. Or you can send in a check with your signed copy of the application & placement agreement.
